- Please contact our office through our online “Request New Patient Appointment” form. Someone from our team will reach out to you within 2 business days to review fees, and services as well as to schedule your first appointment.
- A non-refundable $50 deposit is required to hold your new patient appointment. Your deposit will be applied to the cost of your new visit. Payment is accepted by card ONLY. If your appointment is canceled prior to 3 days, your non-refundable deposit can be applied to a new appointment, provided it is rescheduled within 6 months of the original appointment date.
- Once you’re established as a patient you will be invited to our online patient portal which gives you access to your visit summaries, handouts, and other documents. Comprehensive forms and questionnaires will be sent to your mobile device directly from Phreesia, our patient intake manager.
- 48 hours prior to being seen by the doctor, you will need to complete all our online forms and ensure we have received your appropriate medical records and to allow your doctor to review your records.
Failure to complete your online forms WITHIN 48 HOURS will place you at risk of losing your deposit when your appointment is canceled.
